Referrals

Our system of care is to provide education and therapeutic support to help clients see positive change and experience recovery. If you want to make a referral, please call us at 850.932.9375 or 800.332.2271. We're available 24 hours a day, seven days a week. We'll be happy to answer your questions or arrange an assessment.

To make a referral, please call us and talk with one of our Admissions staff. They will be happy to arrange an assessment or answer any questions.

Please have this information ready when you call:

• Frequency of drug or alcohol use
• Person's attitude about receiving treatment
• Other mental health diagnosis or relevant medical issues
• Insurance verification: name of insurer, policy number, phone number
• Person's information: name, address, date of birth, social security number
  

With the client's permission, our team can follow up with referral sources to provide weekly progress reports. Just let us know how we can serve you best.
