Case Management (Adult)

Case management offers a system of supports and services for people with serious and persistent mental illness or dual diagnosis. Addressing barriers to effective community living, case management provides planning, referrals and advocacy to help clients with transportation, medical and psychiatric care, legal issues, and basic living needs such as housing and food. Case managers work with people in their communities to enhance wellness and quality of life.

Florida Assertive Community Treatment (FACT)
FACT uses a "hospital without walls" approach for people who have serious mental illnesses and are most at risk because of severity of symptoms, low functioning or concurrent substance abuse. Professionals in psychiatry, nursing, counseling, case management, substance abuse treatment and vocational services work as a team to provide a complete set of supports and services that allow clients to safely remain in the community.

Community Recovery Team (CRT)
The CRT represents the most intensive level of care in the case management continuum. The team works closely with people transitioning from acute care or state hospitals, helping them adjust to the challenges of community living.

Traditional Case Management
Traditional case management provides services to help people successfully control their illnesses and maintain the supports and skills necessary for living in the community. Supportive guidance also is provided.
